Preparing for Your Consultation
Making the most of your first appointment with Dr. Stoita starts with good preparation. Please download and complete the patient forms below before you arrive.
First Consultation
What To Bring
- GP referral letter (current, within 12 months)
- Medicare card and private health insurance card
- Completed patient registration and medical history forms
- List of all current medications including supplements and doses
- Any X-rays, MRI, CT scans or ultrasounds related to your condition
- Previous specialist letters, blood tests or surgical notes
- Details of prior injuries, surgeries or relevant chronic conditions
- Known allergies — medications, anaesthetic or latex
- A written list of questions you'd like to discuss
- A support person if you'd like — a partner or family member is very welcome
Fees & Medicare
A consultation fee applies to your appointment. Part of this fee is claimable through Medicare, and your private health fund may also contribute. Please bring your Medicare card and private health insurance card. Payment is required on the day of your consultation.
